      Founded in 1999, St. Hope Foundation is a federally qualified community health center serving Houston and surrounding counties. Over the past 15 years, SHF rapidly grew to becoming a comprehensive community health center with 5 locations. With low cost services on a sliding fee scale, our service lines included: primary medical care, dental care, pharmacy, psychiatry, clinical research and vision care. Recognizing the many healthcare needs within our community, SHF is intently committed to bringing high quality, affordable health care to those in need. Recently, SHF implemented the Patient Centered Medical Home model of care. We serve patients from various disease states that include: Heart disease, Diabetes, Hepatitis C, HIV/AIDS, Obesity, Gout, Mental Health, Cancer and skin disorders. We offer a full range of services.
